The California Guardianship Termination Questionnaire is a form used to determine the eligibility of an individual to terminate a guardianship. This form is typically used when a guardian is ready to resign or when a court has determined that the guardianship should be terminated. There are two types of California Guardianship Termination Questionnaires: a Minor Guardianship Termination Questionnaire and an Adult Guardianship Termination Questionnaire. The Minor Guardianship Termination Questionnaire is used when a guardian of a minor is ready to resign or when a court has determined that the guardianship should be terminated. This questionnaire contains questions regarding the guardian's relationship to the minor, the guardian's plan for the minor, and the minor's current living situation. The Adult Guardianship Termination Questionnaire is used when a guardian of an adult is ready to resign or when a court has determined that the guardianship should be terminated. This questionnaire contains questions regarding the guardian's relationship to the adult, the guardian's plan for the adult, and the adult's current living situation. Both forms must be completed and submitted to the court in order for the guardianship to be terminated.
