The California Application for Psychotropic Medication (CAP) is a form used to initiate the prescribing of psychotropic medications to children ages 0 to 5 years. It is a standardized form developed by the California Department of Health Care Services (DOCS) that must be completed by the prescribing provider to start the process of prescribing psychotropic medications for a child. The form is used to document the clinical indications for the use of the medication, the informed consent process for the use of the medication, and the plans for monitoring the child’s response to the medicationHappyPM is available in three different versions: 1. CAP for infants and toddlers (ages 0 to 2 years): This version of the CAP includes information about the indication for the use of the medication, the informed consent process, and the monitoring plans for the medication. 2. CAP for preschoolers (ages 3 to 5 years): This version of the CAP includes information about the indication for the use of the medication, the informed consent process, and the monitoring plans for the medication. It also includes information about the use of psychosocial interventions and the involvement of the family in the decision-making process. 3. CAP for adolescent and adults (ages 12 and older): This version of the CAP includes information about the indication for the use of the medication, the informed consent process, and the monitoring plans for the medication.
