This document requests that the court grant permission for a conservator to make healthcare decisions, such as authorizing the performance of surgery, on behalf of the person in question. There are two types of California Request For Court Order Authorizing Surgery: Petition and Order Appointing Conservator and Petition and Order Authorizing Surgery.
