The California Supplemental Application For Compensation (Hearing Required) is a form used by individuals seeking compensation from the state of California for physical and/or mental injury and/or disability resulting from a workplace injury. The application is used to request a hearing before the California Division of Workers' Compensation (DWC) so the individual can be heard and receive a ruling on their claim. The California Supplemental Application For Compensation (Hearing Required) can be broken down into two different types: 1. The first type of application is for those who are not represented by an attorney and are seeking to file a claim without legal representation. This application requires the individual to provide information about themselves, their injury and/or disability, the employer, and any witnesses that were present or have knowledge of the incident. 2. The second type of application is for those who are represented by an attorney and are seeking to file a claim with legal representation.
