Colorado Facsimile Cover Sheet is a document used to transmit a fax from one party to another in accordance with the Colorado Rules of Civil Procedure. It is also known as a fax cover sheet. There are two types of Colorado Facsimile Cover Sheet: 1) Colorado Judicial Fax Cover Sheet and 2) Colorado Non-Judicial Fax Cover Sheet. The Colorado Judicial Fax Cover Sheet is to be used when transmitting a fax to the Court or another judicial agency. It includes the sender's name, address, phone number, fax number, and the date of transmission. It also includes a brief description of the purpose of the fax, such as a motion to set the case for hearing, an order to show cause, or a notice of deposition. The Colorado Non-Judicial Fax Cover Sheet is to be used when transmitting a fax to another party in a case, such as an opposing attorney or a witness. It includes the sender's name, address, phone number, fax number, the date of transmission, and a brief description of the purpose of the fax, such as a discovery request or a notice of deposition. Both types of Colorado Facsimile Cover Sheet must be signed by the sender and dated with the date of transmission.
