Colorado Request For A Disfigurement Award is a program administered by the Colorado Department of Health Care Policy and Financing (CPF) that provides financial assistance to individuals who have suffered severe disfigurement due to a traumatic injury or medical condition. The program is designed to help individuals cover the costs associated with reconstructive surgery, prosthetics, and other related medical expenses. There are two types of Colorado Request For A Disfigurement Award: (1) The Major Disfigurement Award, which provides up to $20,000 in financial assistance, and (2) The Minor Disfigurement Award, which provides up to $5,000 in financial assistance. In order to qualify for a Colorado Request For A Disfigurement Award, individuals must be a legal resident of Colorado, be able to provide medical documentation of their disfigurement, and must not have received a disfigurement award from any other source.
