Connecticut Authorization for the Release of Information Re: Application to Restore Right to Purchase, Possess or Transport a Firearm (New 7/11) is a form required for individuals seeking to regain their right to purchase, possess or transport firearms in the state of Connecticut. This form authorizes the release of information about the applicant's criminal history, mental health records, and other relevant information to the Department of Emergency Services and Public Protection (DEPP) in order to determine an applicant's eligibility to purchase, possess or transport a firearm. There are two types of Connecticut Authorization for the Release of Information Re: Application to Restore Right to Purchase, Possess or Transport a Firearm (New 7/11): one for individuals with a criminal record and one for individuals with a mental health record. The form for individuals with a criminal record must be completed and submitted to the DEPP, along with a valid photo identification and a valid Connecticut Certificate of Eligibility.
