The Connecticut Petition/Appointment of Coguardian of the Person of a Minor Child (New 7/15 and Rev. 10/17 re PC-509CI)) is a legal form that is used to appoint an individual or individuals to be jointly responsible for the care and custody of a minor child. This form is available in two versions: the Standard version, and the Limited version. The Standard version is used when a minor child's parents are unable to adequately care for the child on their own. It allows one or two individuals to be appointed as Co-Guardians of the Person of the Minor Child, with the authority to make decisions relating to the child's care and upbringing. It also allows the Co-Guardians to make decisions about where the child will live, and to receive any financial assistance that may be available from state or federal programs. The Limited version is used when a minor child's parents are able to adequately care for the child, but may need additional help or support from a Co-Guardian. This version allows for one individual to be appointed as Co-Guardian of the Person of the Minor Child, with the authority to make decisions relating to the child's care and upbringing. However, the Co-Guardian is not allowed to make decisions about where the child will live or receive any financial assistance that may be available from state or federal programs. Both versions of the Connecticut Petition/Appointment of Coguardian of the Person of a Minor Child (New 7/15 and Rev. 10/17 re PC-509CI) require that the parties filing the petition meet certain criteria, such as proving that the minor child's welfare is in danger or that the parents are unable to adequately care for the child on their own. Once the petition is approved, the Co-Guardian will be responsible for providing physical and emotional care for the minor child.
