The Connecticut Petition/Determination of Competency to Vote RE: Person with Intellectual Disability (Rev. 10/16) is a form used to determine an individual's eligibility to vote in the state of Connecticut. This form is used to determine the eligibility of individuals with intellectual disabilities to vote in the state of Connecticut. The form is designed to assess the individual's understanding of the election process and ability to make informed decisions when submitting their ballot. The form is divided into two parts; the first part is used to determine if the individual has the mental capacity to understand and participate in the election process, and the second part is used to determine the individual's understanding of the election process, their ability to make informed decisions, and their ability to independently cast their ballot. There are two types of Connecticut Petition/Determination of Competency to Vote RE: Person with Intellectual Disability (Rev. 10/16): 1) For individuals with an intellectual disability that is not severe enough to qualify them for supported voting, and 2) For individuals with an intellectual disability that qualifies them for supported voting.
