The form is designed to ensure that anyone enter a public space in the city is following proper hand hygiene and has not come in contact with anyone who has tested positive for the virus. There are three types of District of Columbia Clean Hands Form: Personal Clean Hands Form, Business Clean Hands Form, and Institutional Clean Hands Form. The Personal Clean Hands Form is for individuals who are entering public spaces in the District of Columbia and is intended to document that the individual has properly washed their hands before entering the space. The Business Clean Hands Form is for businesses in the District of Columbia to document their compliance with the DC Department of Health's COVID-19 safety standards. The Institutional Clean Hands Form is for organizations in the District of Columbia to document their compliance with the DC Department of Health's COVID-19 safety standards.
