The District of Columbia Trade Name Registration Form is a document used to register a business name in the District of Columbia. The form is required by the DC Department of Consumer and Regulatory Affairs (DORA) and must be completed and filed with the DORA Corporations Division in order to register a business name in the District. The form requires information such as the business name, the name and address of the owner, the type of business entity, and the purpose of the trade name registration. There are two types of District of Columbia Trade Name Registration Forms: an Entity Trade Name Registration Form, and a Sole Proprietorship Trade Name Registration Form. The Entity Trade Name Registration Form must be completed and filed by corporations, LCS, and other business entities. The Sole Proprietorship Trade Name Registration Form must be completed and filed by individuals who are operating a business as a sole proprietorship.
