The District of Columbia RA-4 Blanket Statement of Change of Registered Office Or Registered Agent is a form used to change the registered office or registered agent of a business entity in the District of Columbia. It is used by domestic and foreign corporations, limited liability companies, limited partnerships, and other business entities. This form must be filed with the District of Columbia Department of Consumer and Regulatory Affairs (DORA) to update the registered office and/or registered agent address. The form consists of two distinct parts: the Statement of Change and the Certificate of Acceptance. The Statement of Change requires the entity to provide the name, address, and type of entity, as well as the address of the new registered office and/or registered agent. The Certificate of Acceptance requires the new registered agent to accept their appointment. There are two types of District of Columbia RA-4 Blanket Statement of Change of Registered Office Or Registered Agent: one for domestic entities and one for foreign entities.
