The District of Columbia Quarterly Premium Surcharge Payment Form is used by employers in the District of Columbia to report and remit quarterly premium surcharges to the Department of Employment Services. The form is used to pay premiums for the District of Columbia Temporary Disability Insurance Program and the District of Columbia Paid Family Leave Program. There are two types of District of Columbia Quarterly Premium Surcharge Payment Form: an Employer Quarterly Report (Form DC-1) and an Employer Payment Form (Form DC-2). Form DC-1 is used to report information about each quarterly period, including the total number of hours paid, the total wages, and the total premium surcharge due. Form DC-2 is used to pay the premium surcharge due, and includes information about the employer's payment, such as the total amount of surcharge due, the payment method, and the payment date.
