Delaware "Proof Service" is a form that is required for health care reimbursement arbitration. The form is used to provide proof of service of the arbitration demand or notice to the opposing party. The form must be completed for all health care reimbursement arbitration cases in Delaware. There are two types of Delaware "Proof Service" forms required for health care reimbursement arbitration: the Delaware Arbitration Notice and Demand Proof Service Form, and the Delaware Arbitration Subpoena Proof Service Form. The Delaware Arbitration Notice and Demand Proof Service Form is used when the opposing party is served with a Notice and Demand for Arbitration. This form requires the name of the party receiving the Notice and Demand, the date of service, the method of service, and the signature of the process server. The Delaware Arbitration Subpoena Proof Service Form is used when the opposing party is served with an Arbitration Subpoena. This form requires the name of the party receiving the Subpoena, the date of service, the method of service, and the signature of the process server. Both forms must be filed with the court after service of the Notice and Demand or Subpoena is completed.
