The Florida Affidavit of Domestic Partnership is a legal document that is used to recognize a domestic partnership in the state of Florida. It is a formal statement that two individuals in a relationship share a common residence and intend to be in a committed and exclusive relationship. The Affidavit of Domestic Partnership can be used to provide evidence of a domestic partnership for legal, financial, and social benefits. There are two types of Florida Affidavit of Domestic Partnership: Affidavit of Domestic Partnership without Financial Obligations and Affidavit of Domestic Partnership with Financial Obligations. The Affidavit of Domestic Partnership without Financial Obligations is used when two individuals in a domestic partnership do not have any financial obligations to each other. The Affidavit must be signed in the presence of a notary public and include the full names, addresses, and dates of birth of both partners. The Affidavit also includes a statement of the intention to share a common residence and to be in a committed and exclusive relationship. The Affidavit of Domestic Partnership with Financial Obligations is used when two individuals in a domestic partnership have financial obligations to each other. This Affidavit includes all the same information as the Affidavit of Domestic Partnership without Financial Obligations, but it also requires the partners to provide additional information related to their financial obligations, such as spousal support, division of property and assets, and other financial matters.
