The Florida Head of Household Affidavit is a document filed in the Sunshine State to declare an individual as the head of the household for taxation purposes. There are three types of Florida Head of Household affidavits: Form DR-462, Form DR-501, and Form DR-502. Form DR-462 is for individuals who are not married but who have dependents living in their home, while Form DR-501 is for those who are married but who have dependents in their home. Lastly, Form DR-502 is for those who are married but are separated and have dependents in their home. Each form requires the individual to provide information such as their name, address, Social Security Number, and the names and ages of any dependents living in their home. The affidavit must also be signed and notarized to be valid. The individual must then file the affidavit with the Florida Department of Revenue to be declared as the head of the household for taxation purposes.
