There are two types of Florida Fact Information Sheet — Individual: a Standard Version and a Revised Version. The Standard Version is used for individuals who have not changed their employment status or residence in the last 3 years. The Revised Version is used for individuals who have changed their employment status or residence in the last 3 years. Both versions provide essential information about the individual and his or her family, such as employment and residence information.
