Georgia Underage Enrollment Affidavit for Home School Students is a document issued by the Georgia Department of Education that allows students who are under the age of six to enroll in a home school program. This document must be signed by the parent or legal guardian of the student, as well as by a representative of the home school program. The affidavit must be notarized, and must include the student’s name, date of birth, and address. There are two different types of Georgia Underage Enrollment Affidavit for Home School Students: one for students who are four years of age or younger, and one for students who are five years of age. The affidavit for four-year-olds must be notarized and include the student’s name, date of birth, address, and a statement from the home school program that the student is physically and mentally able to participate in the program. The affidavit for five-year-olds must also be notarized and include the student’s name, date of birth, address, and a statement from the home school program that the student has the necessary skills and abilities to benefit from the program.
