A Hawaii Statement of Military Service is a document issued by the Hawaii Department of Defense that verifies a person’s military service record. This document is used to validate a person’s service to the state, as well as to provide proof of a person’s eligibility for military benefits. The Hawaii Statement of Military Service comes in two forms: the DD Form 214 and the GB Form 22. The DD Form 214 is issued to all service members upon discharge or retirement from the military, and includes details about an individual’s military service, such as branch of service, dates of service, rank, awards, and decorations. The GB Form 22 is issued to National Guard and Reserve members and includes similar information as the DD Form 214. Both forms are considered official proof of military service, and can be used to apply for veteran benefits.
