The Hawaii Temporary Restraining Order (TO) Questionnaire is a form used by the state of Hawaii to assess whether a person is in need of a restraining order. The questionnaire includes a set of questions about the person's safety, the nature of the abuse, and the person's relationship with the other party. There are three types of Hawaii TO Questionnaire: 1) Domestic Abuse Protective Order (CAPO); 2) Stalking Protective Order (SO); and 3) Harassment Protective Order (HBO). The CAPO questionnaire asks questions about the nature of the abuse, such as whether the respondent is in fear of physical harm, or if there have been threats of physical violence, sexual assault, or stalking. The SO questionnaire asks questions related to the nature of stalking behavior, such as whether the respondent has been followed, surveilled, or threatened. The HBO questionnaire asks questions about the nature of the harassment, such as whether the respondent has been verbally threatened, or if there have been repeated attempts to contact the respondent. Each TO questionnaire also includes questions about the respondent's relationship with the other party, such as if they have any children in common, or if they have ever lived together.
