The Hawaii Request for Transcript and Recording — Non Appeal (Instructions and Form) is a document used to initiate a request for the transcripts and/or recordings of a legal proceeding in Hawaii that is not part of an appeal. This form is used to request a transcript and/or recording of proceedings involving the Hawaii Supreme Court and Intermediate Court of Appeals, the District Courts of Hawaii, the Family Court of the First Circuit, and the Land Court of the State of Hawaii. This form can also be used to request transcripts and/or recordings of proceedings in the Circuit Court of the Second Circuit, the Third Circuit, the Fifth Circuit, and the Sixth Circuit. There are two types of Hawaii Request for Transcript and Recording — Non Appeal (Instructions and Form): one for Transcripts only and one for Transcripts and Recordings. Both forms include instructions on how to complete the form, and contact information for the court or agency from which the transcript and/or recording is requested.
