The Hawaii Application Form (VOLUNTARY PROTECTION PROGRAM (VPP)) is a form used by employers in Hawaii to apply for participation in the Voluntary Protection Program (VPP). The VPP is a safety and health program that recognizes and promotes effective workplace safety and health management. The VPP is administered by the Hawaii Occupational Safety and Health Division (HIS) and is aimed at reducing occupational injuries, illnesses, and fatalities. The Hawaii Application Form (VOLUNTARY PROTECTION PROGRAM (VPP)) includes two types of forms: the Initial Application Form and the Annual Certification Form. The Initial Application Form is used to apply for participation in the VPP and includes information about the employer, the worksite, and the health and safety management system. The Annual Certification Form is used to certify that the employer is meeting the VPP requirements and includes information about workplace safety and health activities, inspections, and training.
