Iowa Groundwater Hazard Statement Form Attachment #1 is a two-page document used to provide information to the Iowa Department of Natural Resources (DNR) about areas of the state where there is a potential risk of groundwater contamination. This form is used to assess the potential hazard to groundwater and to provide information that can be used to develop strategies to protect the resource. The form includes two types of attachments: 1) A Hazard Statement form which provides information about the location and characteristics of the potential hazard, and 2) a Risk Assessment form which outlines the risk posed to the groundwater from the potential hazard. Both of these forms must be completed and submitted to the DNR for review and consideration.
