The Application for Reinstatement of an LLC [499.78B] is used by LCS to apply for reinstatement of their rights, privileges, and powers that have been suspended or revoked due to a violation in Iowa. Both applications require the applicant to provide information such as the name of the entity, the address of the entity, the date of the suspension or revocation, a list of the violations that led to the suspension or revocation, and a detailed explanation of the changes or corrective actions that have been taken to prevent future violations. The application must be accompanied by a filing fee and any other documentation required by the Iowa Secretary of State.
