There are three types of Iowa Statement of Cancellation: Cancellation of a Service Agreement, Cancellation of a Home Solicitation Sale, and Cancellation of an Automobile Sale. The Cancellation of a Service Agreement is used when a consumer is cancelling a service contract, such as for repairs, installation, or maintenance. The Cancellation of a Home Solicitation Sale is used when a consumer is cancelling the purchase of goods or services that were solicited at a consumer's home, place of employment, or dormitory. The Cancellation of an Automobile Sale is used when a consumer is cancelling the purchase of a vehicle. All of these documents must be filled out and signed in order to be legally binding. The Iowa Statement of Cancellation is also accompanied by a Notice of Cancellation that must be sent to the vendor or supplier within three business days of signing the form. The Notice of Cancellation should include the consumer's name, the vendor's name and address, and the date of the cancellation.
