The form is used to determine if an individual meets the criteria for a disability or impairment. There are three types of Idaho Statement of Examining Physician: 1. Physical Impairment Report: This form is used to document a physical impairment, such as an injury or illness, and the results of a physical examination relevant to the impairment. 2. Mental Impairment Report: This form is used to document a mental impairment, such as a mental disorder or illness, and the results of a mental health examination relevant to the impairment. 3. Combined Physical and Mental Impairment Report: This form is used to document both physical and mental impairments, and the results of a physical and mental examination relevant to the impairments.
