The Statement of SSN issued to individuals who have a valid SSN issued by the SSA includes the individual’s name, date of birth, and SSN. The Statement of SSN issued to individuals who do not have a valid SSN issued by the SSA includes the individual’s name, date of birth, and a randomly generated number provided by the Idaho Department of Health and Welfare.
