The Idaho Certificate of Surrender is a document issued by the Idaho Department of Insurance that allows a policyholder to voluntarily surrender their life insurance policy. The policyholder must fill out the form and submit it to the insurance company with the original policy and any accumulated dividends. There are two types of Idaho Certificate of Surrender forms: an individual form for the surrender of an individual life insurance policy, and a joint form for the surrender of a joint life insurance policy. The form must include the policyholder's name, address, date of birth, policy number, and signature of the insured or authorized representative. The form must also include a signed statement from the policyholder or authorized representative releasing the insurer from any and all obligations under the policy and providing the insurer with the right to cancel the policy. Upon receipt of the form, the insurer must issue a cash surrender value check to the policyholder or authorized representative.
