The Idaho Affidavit Verifying Income (H&W) is an official document used to prove an individual's income. It is used by the Idaho Department of Health and Welfare (H&W) for the purpose of determining eligibility for various health and welfare benefits. The affidavit is completed by the applicant and signed by both the applicant and a witness. It requires basic information such as the applicant’s name, address, and Social Security Number; the number of dependents; the type of income being claimed; and the amount of income being claimed. The affidavit must also include a statement from the applicant that the information is accurate and true. It must be notarized to be considered valid. There are three types of Idaho Affidavit Verifying Income (H&W): the Affidavit of Self-Employment, the Affidavit of Employment, and the Affidavit of Social Security Benefits. The Affidavit of Self-Employment is used by individuals who are self-employed and must provide evidence of their income. The Affidavit of Employment is used by individuals who are employed by a company and must provide evidence of their income. The Affidavit of Social Security Benefits is used by individuals who receive Social Security benefits and must provide evidence of their additional income.
