Idaho Name Change (Adult and Minor) is the process of legally changing the name of an individual in the state of Idaho. An adult may change their name for any reason, while a minor may only change their name if it is in the best interests of the child. Adult name change: An adult may file a Petition for Name Change in the district court of the county in which they reside. The Petition must include the individual's current name, the proposed name change, the reason for the name change, and the individual's signature. The Petition must also be signed by a witness who is not related to the petitioner. Minor name change: A minor may file a Petition for Name Change in the district court of the county in which they reside. The Petition must include the minor's current name, the proposed name change, the reason for the name change, and the signatures of both the minor and their parent(s) or guardian(s). The Petition must also be signed by a witness who is not related to the petitioner. Once the court approves the Petition, the individual will receive an official court order for the name change. The individual must then submit the court order to the Idaho Bureau of Vital Records to obtain a new birth certificate and other legal documents reflecting the new name.
