Idaho Out-of-State Check Writing, Application and Affidavit is a form that is used by businesses and individuals located outside of Idaho to write checks and conduct business transactions within the state. This form must be completed and submitted to the Idaho State Tax Commission in order to be eligible to write checks in Idaho. It includes a section for the applicant to provide their personal information and business information, as well as a section for the signature of the applicant and a witness. There are two types of this form: the Idaho Out-of-State Check Writing Application and the Idaho Out-of-State Check Writing Affidavit. The Application is used to establish the applicant's eligibility to write checks within the state, while the Affidavit is used to provide further evidence of the applicant's eligibility to write checks. Both forms must be completed and submitted to the Idaho State Tax Commission in order to complete the application process.
