The Illinois Affidavit of Title — Bill of Sale is a document that is used to transfer ownership of a motor vehicle from a seller to a buyer. It is used when the vehicle's title is not available, or when the title does not match the information on the vehicle. The Affidavit of Title — Bill of Sale must include the following information: seller's name and address, buyer's name and address, description of the vehicle (year, make, model, body type, vehicle identification number (VIN), and odometer reading), sale price, date of sale, and seller's signature. In addition, the seller must include a notarized statement verifying that they are the true owner of the vehicle. There are two types of Illinois Affidavit of Title — Bill of Sale: the Certificate of Title Without Assignment Affidavit, and the Certificate of Title With Assignment Affidavit. The Certificate of Title Without Assignment Affidavit is used when the seller is the registered owner of the vehicle and is transferring ownership without an intermediary. The Certificate of Title With Assignment Affidavit is used when there is an intermediary (e.g., a dealership or another individual) involved in the sale.
