Illinois Admission of Parentage is a legal document that establishes a parent-child relationship when the unmarried parents have not signed a Voluntary Acknowledgment of Parentage. This document is signed by the parent and witnessed by a notary public. It is filed with the Illinois Department of Healthcare and Family Services (HFS). There are two types of Illinois Admission of Parentage: one for a child born in Illinois and one for a child born outside of Illinois. For a child born in Illinois, the parent must sign the Admission of Parentage form in the presence of a notary public and submit the form to HFS. For a child born outside of Illinois, the parent must sign the Admission of Parentage form in the presence of a notary public and submit the form to HFS along with a certified copy of the child’s birth certificate. After the Admission of Parentage form is filed, HFS will issue a Certificate of Parentage to the parent.
