The Illinois Petition For Appointment Of Guardian (Disabled Adult) is a legal document used to request the appointment of a guardian to manage the affairs of an incapacitated adult in Illinois. This document must be filed in the county court where the adult lives or has a legal residence. The Petition must include the full name, address, and date of birth of the disabled adult, as well as the proposed guardian’s name, address, and relationship to the disabled adult.
