The Illinois Income and Asset Form is a document used by the Illinois Department of Human Services (DHS) to determine a person's eligibility for financial assistance programs such as Supplemental Nutrition Assistance Program (SNAP), Temporary Assistance for Needy Families (TANK), and Medical Assistance. There are two types of Illinois Income and Asset Form: an Income and Assets Form (I&A) and a Modified Adjusted Gross Income (MAGI) Form. The I&A Form is used for households that do not have a valid Social Security number. The MAGI Form is used for households that have a valid Social Security number.
