The Illinois Admission of Paternity is a form signed by a man who acknowledges that he is the biological father of a child born out of wedlock. This form is required when the parents are not married and the father wants to establish legal rights and obligations to the child. There are two types of Illinois Admission of Paternity: Voluntary Acknowledgement of Paternity and Judicial Acknowledgement of Paternity. The Voluntary Acknowledgement of Paternity is a form signed by both the mother and the father and filed with the Illinois Department of Healthcare and Family Services. The Judicial Acknowledgement of Paternity is a form signed by the father and filed with the circuit court in the county where the child was born. Both forms are used to establish legal paternity of a child and provide the father with legal rights and obligations to the child.
