The Illinois Adult Sentencing Judgment/Probation Order (Long Form) is a legal document issued by the state of Illinois to persons convicted of a criminal offense. The document sets forth the terms of the offender’s sentence, including the type and length of probation, any fines or restitution to be paid, and any special conditions of probation. The document also may include an order of commitment to the Department of Corrections, if applicable. The Long Form is used when the court imposes a sentence that includes probation or conditional discharge. There are two types of Illinois Adult Sentencing Judgment/Probation Order (Long Form): the Standard Long Form and the Extended Long Form. The Standard Long Form is used when the offender is not placed on probation or conditional discharge for more than two years. The Extended Long Form is used when the offender is placed on probation or conditional discharge for more than two years.
