The Illinois Order for Release of Educational Records (also known as the Student Records Transfer Form) is a document that is used to transfer student educational records from one school to another. It is commonly used when a student is transferring from one school district to another or when a student is transferring from one school to another within the same district. The form must be completed by the student and signed by both the student and the student’s parent or guardian. It provides information about the student such as name, address, date of birth, school attended, grade level, etc. The form also provides the name, address, and phone number of the school from which the student is transferring, as well as the name, address, and phone number of the school to which the student is transferring. It also includes a section for the student to indicate which educational records they wish to have released, such as grade reports, transcripts, discipline records, and special education records.
