It is filed with the Illinois Appellate Court to provide the court with information about the parties involved, the issue being appealed, and the lower court’s decision. The cover sheet also includes a payment of appellate court costs and a signature from the party initiating the appeal. There are two types of Illinois Administrative Review Cover Sheet, one for filing an appeal in the Circuit Court and one for filing an appeal in the Appellate Court. The Circuit Court Cover Sheet is used to initiate an appeal of a final administrative decision made by an administrative agency. The Appellate Court Cover Sheet is used to initiate an appeal of a final decision made by either a Circuit Court or an Administrative Agency.
