Guardian’s Notice To Court and Parents Prior To Removing Minor From The State of Illinois For More Than 48 Hours But Less Than 30 Days is a document that guardians must file with the court before temporarily taking a minor out of the state for more than two days but less than one month. This document is required by the Illinois Supreme Court when a guardian intends to take a minor from the state. This document must be filed with the court at least 30 days before the guardians intend to take the minor out of the state. The name and address of the guardian; 2. The name and address of the minor; 3. The proposed date of departure and the proposed date of return; 4. The name and address of the person with whom the minor will be staying; 5. The reason for the removal from the state; 6. The name and address of the person who will be responsible for the minor’s care while away; 7. The name and address of the person to whom the guardian will send reports on the minor’s health, education, and welfare; 8. A statement that the guardian will return the minor to the state as soon as the reasons for the removal from the state no longer exist; 9. A statement that the guardian will not allow the minor to remain outside the state for longer than 30 days without obtaining permission from the court; 10. A statement that the guardian has notified the minor’s parents or other interested parties of the proposed removal from the state; and 11. The signature of the guardian. Once the Guardian’s Notice To Court and Parents Prior To Removing Minor From The State of Illinois For More Than 48 Hours But Less Than 30 Days is complete, the guardian must file it with the court. Once the court has received the document, it will determine if the removal of the minor from the state is in the minor’s best interest. If the court approves the removal, the guardian will be allowed to take the minor out of the state for the designated period of time.
