The Illinois Authorization To Cancel Electronic and Bank-By-Mail Direct Deposit (ATTEND) is a form that allows individuals to cancel an electronic or bank-by-mail direct deposit of their salary, wages, or other income. This form must be completed and submitted to the employer, payroll provider, or other payer in order for the direct deposit to be canceled. The form is available online and can be printed out and filled out manually or filled out electronically. There are two types of ATTEND forms: the Authorization To Cancel Electronic Direct Deposit (IATCEDD-EDD) and the Authorization To Cancel Bank-By-Mail Direct Deposit (IATCEDD-BBM). The IATCEDD-EDD form is used to cancel an electronic direct deposit of salary, wages, or other income, while the IATCEDD-BBM form is used to cancel a bank-by-mail direct deposit of salary, wages, or other income. Both forms require the employee or individual to provide their name, address, and Social Security Number, as well as the name and address of the employer, payroll provider, or other payer. The forms also require the employee or individual to sign and date the form in order for it to be valid.
