The Illinois Civil Complaint for Civil Rights Violations (For Use By Non-Prisoners) is a form that non-prisoners can use to file a complaint in the state of Illinois alleging civil rights violations. This form was created by the Illinois Department of Human Rights (DHR) in order to protect the civil rights of Illinois residents. The form allows non-prisoners to make a complaint if they feel their civil rights have been violated due to discrimination, harassment, or other violations of their civil rights. The Illinois Civil Complaint for Civil Rights Violations (For Use By Non-Prisoners) is divided into two parts. Part one is a general description of the alleged civil rights violation. This includes the type of civil rights violation, the date and location of the alleged violation, and any other pertinent information. Part two is a detailed account of the alleged civil rights violation, including any supporting evidence. There are two types of Illinois Civil Complaint for Civil Rights Violations (For Use By Non-Prisoners): the Complaint for Employment Discrimination and the Complaint for Discrimination in Public Accommodations. The Complaint for Employment Discrimination is used to file a complaint when an individual believes they have been discriminated against in the workplace. This includes discrimination based on age, race, gender, disability, religion, or national origin. The Complaint for Discrimination in Public Accommodations is used to file a complaint when an individual believes their civil rights have been violated in a public place or facility. This includes discrimination in places such as restaurants, hotels, transportation, and entertainment venues. The Illinois Civil Complaint for Civil Rights Violations (For Use By Non-Prisoners) must be completed accurately and submitted to the DHR within 180 days of the alleged civil rights violation. Upon receiving the complaint, the DHR will investigate the complaint and take the appropriate action.
