Illinois Getting Started-Request for Certificate of Good Conduct is a request form for Illinois residents that allows them to submit a request for a Certificate of Good Conduct. This certificate is used to demonstrate to employers, educational institutions, or other governmental entities that a person has not been convicted of any criminal offenses in the State of Illinois. The form must be completed and signed by the individual requesting the certificate. There are two types of Illinois Getting Started-Request for Certificate of Good Conduct: the Request for Certificate of Good Conduct and the Request for Certificate of Nonconviction.
