The Illinois Request for Certificate of Good Conduct is a document issued by the Illinois State Police that is used as an official statement of a person’s criminal history. It is also known as an Illinois State Police Record or a Certificate of Good Conduct. The Certificate is used to provide information to employers or educational institutions about a person’s criminal history. It typically contains information about arrests, charges, convictions, and sentencing related to criminal offenses. There are two types of Illinois Request for Certificate of Good Conduct: a Basic Certificate and an Expanded Certificate. The Basic Certificate includes information regarding arrests, charges, and convictions within the State of Illinois. The Expanded Certificate includes information from other states as well as from the FBI. Both types of Certificates are valid for a period of two years. To apply for an Illinois Request for Certificate of Good Conduct, the applicant must submit a valid form of identification, a completed application form, and the applicable fee. The application can be submitted online or by mail. Once the application is submitted, the State Police will process the request and issue a Certificate of Good Conduct.
