The Illinois Living Wage Ordinance affidavit is a form required by the state of Illinois for businesses to certify that they are paying their employees at least the minimum wage mandated by the state. The Illinois Living Wage Ordinance requires employers to pay employees a wage of at least $11.00 per hour as of July 1, 2020. Employers must submit an affidavit to the Illinois Department of Labor verifying that they are in compliance with the state's living wage requirements. Depending on the size of the business, there are two types of affidavits required: (1) the General Affidavit, which must be completed by all employers, and (2) the Annual Wage Notice, which must be completed by employers with more than 10 employees. Both affidavits must include the employer's name, address, and contact information, as well as the name and wages of each employee. After the employer has signed the affidavit, it must be submitted to the Illinois Department of Labor for approval.
