Illinois Incentives Class Living Wage Ordinance affidavit is a document used by employers to affirm that they are paying their employees a living wage. The living wage is determined by the state of Illinois and varies from county to county. The affidavit must be completed and submitted to the Illinois Department of Labor when applying for the Illinois Incentives Class. There are two types of Illinois Incentives Class Living Wage Ordinance affidavits: a standard affidavit and an affidavit for employees receiving tips. The standard affidavit must be signed by the employer and affirms that employees are being paid at least the minimum wage as set forth by the county for which the business is located. The affidavit for employees receiving tips requires the employer to affirm that employees are being paid at least the minimum wage, plus the additional amount required for tipped employees. In addition, the employer must certify that employees are receiving at least the minimum hours of work required by the county for the job they are performing. Both affidavits must be notarized and are subject to audit by the Illinois Department of Labor.
