Illinois Request for Name Change (adult) is a legal document used to officially change an adult's name in the state of Illinois. This document is filed with the Clerk of the Circuit Court of the respective county, where the applicant resides. In order to apply for a name change, the applicant must meet certain criteria and provide the required documents. There are two types of Illinois Request for Name Change (adult): Voluntary Name Change and Court Order Name Change. Voluntary Name Change is requested by the applicant and does not require a court order. Court Order Name Change requires a court order to be granted. In either case, the applicant must complete the Request for Name Change form, provide proof of identity, pay the applicable filing fee, provide two affidavits from two witnesses, and submit a newspaper notice of the name change. After all the required documents are verified, the Clerk of the Circuit Court will issue the Certificate of Name Change.
