Illinois Request for Name Change (Minor Children) is a legal process for changing the name of a minor (under 18 years of age) living in the state of Illinois. The process requires filing a Petition for Name Change with the Circuit Court Clerk in the county where the minor resides. The Petition must specify the reason for the name change request and must be notarized before it can be filed with the Court. Once the Petition is filed, the Court will set a hearing date, at which the parent or guardian must appear and present evidence in support of the name change. If the Court approves the request, a final order will be issued which legally changes the minor's name. There are two types of Illinois Request for Name Change (Minor Children): Regular and Simplified. The Regular Request for Name Change requires the parent or guardian to file a Petition with the Circuit Court Clerk and attend a hearing before the Court to obtain the name change order. The Simplified Request for Name Change allows for an uncontested name change without a hearing, if the parent or guardian has the written consent of the other parent or legal guardian of the minor.
