The Illinois Affidavit as to Military Service is a document created by the Illinois Department of Veterans Affairs (IDEA) to confirm an individual's military service record. It is used to verify an individual's eligibility for certain benefits and services, such as Veterans Preference in Civil Service Employment, and can also be used for obtaining a military service medal. There are two types of Illinois Affidavit as to Military Service: Active Duty and Reserve/National Guard. The Active Duty form is for individuals who have served in the regular armed forces and the Reserve/National Guard form is for individuals who have served in the reserves or the National Guard. Both forms must include the individual's name, date of birth, branch of service, dates of service, and any decorations or awards received. The individual must have their signature notarized in order for the affidavit to be valid.
