The Illinois Getting Started-APPlication for Waiver of Court Fees (appellate Court) is an online form that allows individuals to apply for a waiver of fees for appeals in Illinois courts. It is available to individuals who cannot afford to pay the court fees associated with filing an appeal. The application consists of two separate forms, the Application for Waiver of Court Fees and the Financial Information Statement. The Application for Waiver of Court Fees form requests information such as the name and address of the appellant, the name of the court and the case number, the type of fee to be waived, and the reason for waiving the fee. The Financial Information Statement requires the applicant to provide detailed information about their income, assets, and expenses. Once the forms are completed and submitted, the appellate court will review the application and make a determination on whether to grant the waiver. If the application is approved, the applicant will be able to file their appeal without having to pay the associated fees.
